ABSTRACT

Desmoplastic trichoepithelioma is a variant of trichoepithelioma with distinct histopathologic features. It is characterized by cords and nests of basaloid cells and keratinous cysts surrounded by a desmoplastic stroma. Clinically it appears as a small plaque with a depressed center, mainly located on the face of young females. The main clinical and dermoscopic differential diagnosis consists of morphea-like basal cell carcinoma. Desmoplastic trichoepithelioma frequently displays linear or arborizing vessels and a whitish background at dermoscopic examination. Moreover, milia-like cysts are frequently seen, which correspond to keratinous cysts.
