ABSTRACT

An electronic health record (EHR) is an individual patient’s medical record stored in digital format. It can be viewed as a longitudinal report that can be generated in one or more encounters in any care delivery setting. The EHR can contain several types of patient data, such as the patient’s demographic information, clinical data such as vital signs, medical history, immunizations, laboratory and radiology data, problems and progress notes, accounting and billing records, and even legal documents such as living wills and health powers of attorney.
