ABSTRACT

This chapter focuses on the use of incident databases at the company level. An incident database plays an important role in experience feedback as a compilation of incident experience from different sources. The stored incident data typically consist of facts about the incident, including time and place, activity, the sequences of events, type of incident, and consequences. The database may also include remedial actions and status of implementation. The chapter elaborates the model of a safety information system. It discusses the principal difference between qualitative or free-text data on the one hand and coded data on the other hand. Uni- and bi-variate distribution analyses use coded data. Severity-distribution analysis can be used in estimations of the probability of severe incidents at a workplace and in comparing different workplaces with respect to the expected severity of incidents. In industrial safety, incident concentration analysis is carried out in several dimensions.
