ABSTRACT

The person living with dementia may require increased care and support over time, and the care partner may or may not be able to provide support to match increased needs. This chapter considers the different ways a person may require increased support and care, whether this is at home in the community, in hospital or through an admission to a care home. Changes in care needs require further adjustment and can be distressing to both the individual living with dementia and to their care partners, particularly if their initial hopes for support and care cannot continue with the reality of the progression of the dementia or complexities arising from other health or social circumstances.
