In the event, Hewlett-Packard identified the following reasons for the problem:

Some people couldn’t load the toner cartridge properly or deal with minor problems that arose

Some dealers couldn’t sort out minor problems

Some toner cartridges were being sent to unauthorised refilling companies, which were refilling the same cartridges up to five times so they were wearing out, and the toner in the refilled cartridges was not up to Hewlett-Packard’s normal standards.
