ABSTRACT
Starting this chapter has been as difficult as finding a fault on a complex system. The old adage of ‘start at the beginning and finish at the end’ is all right for story tellers or authors of technical books, but is not always sensible for fault finders! Perhaps it would be wise to begin by detemining what exactly a fault is.
