ABSTRACT

Accidents happen everywhere: in the home, on the road, in sporting events; the world has never been a completely safe place. But we expect a place of work to be a controlled environment, where everyone has a defined role to play, and which operates according to a set of rules. Here, if anywhere, the actions of people, and events, should be regulated and well-organized. And yet, alas, accidents at work continue to cost hundreds of lives each year, and thousands of injuries.
