ABSTRACT

This chapter provides a practical guide to straightforward diagnostic criteria, where exist of some of the conditions that constitute a Long-Term Condition (LTC) such as diabetes, asthma, Chronic Obstructive Pulmonary Disease (COPD) etc. The conditions have more complex diagnostic criteria such as cardiovascular disease (CVD), epilepsy, arthritis etc. The criteria used are those currently recommended in the UK, and are generally based on the National Institute for Health and Care Excellence (NICE) guidance and Clinical Knowledge Summaries (CKS) which provides primary care practitioners with readily accessible summaries of current evidence and clinical guidance on best practice. Hypertension is the most important preventable causes of premature morbidity and mortality in the UK. The chapter provides an overview of diagnostic criteria and signs and symptoms associated with the LTCs that often present in primary care.
