The basic principle of the Mainz pouch II involves the construction of a continent rectal reservoir of high capacity and low pressure with antirefluxive ureteral implantation. This was mentioned first by Kocher in 1903 and clinically implemented by Fisch in 1991.1

The Mainz pouch II is the method of choice for continent urinary diversion in patients with anatomic or functional loss of the bladder who have a competent anal sphincter. In case of ureter obstruction in ureterosigmoidostomy or to change the incontinent colon conduit in a continent form of diversion, conversion in a Mainz pouch II can be performed.
