ABSTRACT

In this chapter I want to stand back from the issue of how the organisation of hospital services impacts on child and family adjustment and explore instead how people can be encouraged to work together to meet the needs of children and their families, once they have been discharged back to their local community. This is a very important area in the treatment of chronically-ill children because the majority of ongoing care is provided in the community, and therefore systems need to be in place to ensure that interventions started in hospitals dovetail into the local provision.
