Deciding which patients should undergo coronary revascularisation remains a key challenge in the management of coronary artery disease, with practice patterns varying widely (Selby et al. 1996). Well-designed expert panels can closely reflect the views of practising physicians (Ayanian et al. 1998), and methods for detecting the underuse of revascularisation are highly reproducible (Shekelle et al. 1998).
