ABSTRACT

The purpose of partnership working in cases is to ensure that a person’s needs can be properly assessed, and that the right services are provided – and it cannot be done in the absence of partnerships. An assessment and care plan for someone with acute mental health problems will involve a social care worker spending time with the person needing a service, discussing the person with family and/or friends, in order to understand their perspective on what has been happening, and discussing the person’s needs and any risks with professionals like a GP. To be successful, those conversations have to take place within a partnership framework if all involved are to be put at ease and to feel they are making a contribution to the potential service user’s health and well-being.
