Errors in design and planning are treated elsewhere (Taylor 1979). The material in this chapter covers errors in operation, maintenance and repair.

Operator error is inevitable in system design. The operator, like any other component in a process plant has a certain capacity .. When demands on the operator exceed his capacity, the result will be failure or error. The operator also inevitably has a stochastic aspect to his behavior. If the system cannot tolerate the occasional randomness in his responses, the result will be an error.
