Compound and complex crises

The causes of any major disaster – such as an air crash or a train wreck – are complex, in that they usually require a sequence of events and

decisions before they occur. Often, a mechanical malfunction along with human error, plus a flaw in the “defense” mechanisms of a complex system, are all required for system failure – and disaster – to occur.
