ABSTRACT

These studies raise serious doubts about the way procedure systems are designed and operated at many nuclear facilities. The purpose in this chapter is to review the common problems associated with procedure systems to determine what, if anything, can be done to improve the quality of procedure provision in a nuclear setting. It is suggested that procedure-related failures largely have their origins in weaknesses inherent within the infrastructure of the organisation. Consequently, it seems reasonable to suppose that it is only by making interventions at this level that we can possibly hope to reduce the overall numbers of incidents that involve a procedure component.
