Heart failure has reached epidemic proportions in the developed world, creating a substantial challenge to health care systems both medically and economically. The combination of aging of the population and improved survival after acute myocardial infarction has resulted in a substantial increase in the prevalence of chronic heart failure, a trend that seems likely to continue for the foreseeable future.1
