3.2 Issues not included in the review

The literature on ``stress'' has not been included; the complexities of de®nition and measurement render interpretation very dif®cult and there is no direct or clear relationship to the common mental disorders. Severe mental illness, psychosis, has not been reviewed as numbers are very small, and different research strategies are required. Studies of treated populations raise important issues of availability of services, variation in professional practice, and illness behaviour, but cannot be interpreted in terms of population prevalence.
