ABSTRACT

Figure 7.1d After mechanical lysis by balloon angioplasty: residual total occlusion of a first septal branch and restoration of flow in the two principal branches of the LAD.

Conventional balloon angioplasty is an outdated approach for proximal LAD occlusion. The achievement of an outward ‘normal’ flow in the epicardial branches after macroembolization is generally associated with poor or absent myocardial salvage due to extensive microvessel disruption.
