ABSTRACT

The era of safety management by the support of safety management systems is relatively new and it is considered to be driven by the findings of the Challenger and Chernobyl accident investigations (Hale, 2003; Hollnagel, 2014). It has entered the domains of the land-based industry, the aviation industry and the maritime industry alike. The objective of safety management systems is to reduce and prevent accidents of managerial origin, by introducing managerial and organizational methods (Okstad & Hokstad, 2001).
