ABSTRACT

Preprinted forms are advantageous because they provide a preset scope of data collection, consistent data collection with low incidence of missing data, and ease of reviewing and reconstructing data. When all surveyors or auditors use the same forms, data collection problems are minimized. The team notebooks containing the preprinted audit forms are filled out as the teams move through the building, using one form for each space. Keeping these forms in the same order as the teams go through the building has proved valuable to fill gaps in information discovered when doing the computer entries on-site. Several types of survey and audit forms are provided. They are provided for field surveyors and auditors who are encouraged to copy or modify them to fit their needs. These forms have been field-tested and have proved to provide efficient, consistent data collection. Checklist is a lighting survey checklist used for a walk-thru survey to determine retrofit potential.
