W om en were screened for interest in the study and for eligibility by telephone. If eligible and interested, they received a copy of the baseline questionnaire in the U.S. mail to com plete at home. They mailed the com pleted questionnaire to the research institute in a stamped, addressed envelope mailed with the questionnaire. If study staff did not receive an envelope within 2 weeks, the woman received a rem inder call. An opportunity to com plete a subsection of the baseline questionnaire was offered to women who did not respond to the rem inder call. Seventy-three women completed the baseline questionnaire by mail, and 43 com pleted the questionnaire subsection by telephone.
