ABSTRACT

Like the assessment of the Minotaure reactor, the second case we studied

concerned a research facility operated by the CEA and named Artémis (cf. Sidebar 14). However, in this case, the human factors experts’ assessment was quite different in terms of the resources used and its conclusions. Following an in-depth investigation of two incidents that occurred in 2004 and 2005, the expert mapped out the chronology and the “causal relationship” of each. He expressed his opinion on the progress of the corrective actions the licensee had undertaken to implement and proposed others.
