There is often a great deal of confusion about care plans, how they should be written (and by whom), their purpose and how long they should be in place. It needs to be clear that the care plans highlighted in the Pocket Guide are not long term or the type that feature in the Care Planning Approach (CPA) documentation. These are short-term, focused on specifi c risk issues and designed to be evaluated and adapted as a response to small but important changes, in the context of risk either increasing or decreasing. There are specifi c interventions to be followed by patient and clinical staff.
