ABSTRACT

In this chapter we return to the crucial issues of care planning and goal setting. A formulation and structured evidenced-based intervention is of little use if it is not carried out or if there is no clear way to measure progress, or a lack of progress which might necessitate a reformulation. In this chapter we illustrate how the service level formulation can be usefully translated into effective care plans and goals. As noted in Chapter 2, care plans are often poorly devised and implemented, which can result in interventions being carried out inconsistently or not at all. Consider the following care plan for Claire:

1 Support Claire to improve her personal hygiene. 2 Improve adherence to medication. 3 Encourage contact with the family. 4 Support Claire to engage in social activities. 5 Manage Claire’s risk of self-harm.
