ABSTRACT

Assessment and care planning are a continuing process and a foundation for good clinical practice. They are a fundamental component within the framework of the systematic approach to the provision of care and interventions. Models of Care (NTA 2002) place emphasis on assessment, care planning and co-ordination of care. For a systematic approach to health and social care interventions, evaluation of care should be an important component of the process of care. It is stated that all those who enter into structured drug and alcohol treatment services should receive a written care plan, agreed with the client (NTA 2002). The progression through the drug treatment system has been described as the client journey (Audit Commission 2004) and covers all stages of a person’s involvement with services, from referral discharge and continuing care.
