The aim of this chapter is to draw together some of the issues that have now been discussed throughout this book and put them into practice through a case study approach. Case scenarios will be provided relating to various conditions, beginning with straightforward cases and progressing to more complex issues. Although based on actual patients, all names and identifying details have been anonymised to ensure confidentiality. This chapter provides both straightforward and more complex case scenarios for the reader to work through, relating to some of the longterm conditions (LTCs) seen in primary care. Part One considers care management issues for people with a specific LTC. Part Two considers care management issues for those with more complex needs. As in earlier chapters, pauses for reflection are provided for each scenario. A suggested course of action is then supplied for each scenario. In practice each person’s individual health and cultural and social needs should be taken into consideration when planning care.
