My introductory quotation from this chapter comes from a medical magazine, and is written by a doctor. The subject matter, the Lancet outcome study of patients treated at the Bristol Cancer Centre, is probably now only of historic interest. But the quotation emphasises why this chapter is necessary. It is pithy, eloquent, inspiring and usually wrong.
