ABSTRACT
The six-page questionnaire we sent out was printed on both sides of a sheet 25. 5 inches wide and 11 inches high. The sheet was creased so that when the righthand third was folded in toward the center, and the left-hand third folded on top of it, the questionnaire was a standard 8.5" × 11" in size. When respondents opened it, they found page 1 on the front, pages 2, 3, and 4 on the inside, and pages 5 and 6 on the back.
