ABSTRACT

This chapter will focus on the written elements of the prescriptive process, including the elements of a well-written prescription and the records necessary to document prescriptive practice.

The written prescription

A patient comes to a prescriber for the prescriber’s knowledge, judgment, evaluation, and thinking. The end product of these endeavors is usually a written prescription. The essential elements, without which a prescription cannot be valid, are listed in Table 21.1. Additional elements that may or may not be included in a prescription are in Table 21.2.
