We might claim to have developed good procedures and systems and might be following them religiously; however, we cannot forget that we are human beings, have a tendency to relax and forget vital things unconsciously. We, unlike machines, get tired and use our own logics while following instructions or procedures and tend to bypass parts of system feeling it as not essential. It also might be a fact, that a particular activity was really not essential to get our results and meeting objectives without problems. In a number of cases, we are unable to identify the inadvertent mistakes or deviations while we are on the job, but can realize when others point it out. It is essential that there is a system of monitoring.
