ABSTRACT

A young orphan is brought by a carer to the emergency department with a painful right leg. He looks hungry and malnourished and after scoffing down a sandwich askes you ‘please sir can I have some more?’ He lets you examine him. He has bruises all over his chest and arms and a very painful swollen leg. His carers are unsure how long he has been like this and do not remember any injury. The carers are quite evasive and want you to give the all clear quickly and become a bit annoyed when you say you need to do some tests. You obtain a radiograph of the pelvis and legs ( Fig. 19.1 ).
