ABSTRACT

Clinicians were able to view patients’ medication lists in the electronic health records (EHR), and nurses used the EHR’s medication administration record capability. In addition, the lack of build-out of the convenience order sets in the pharmacy module caused delays in order entry, pharmacy verification, medication dispensing, and medication administration. To ensure the hospital “got it right” for patients, it was crucial to rebuild the trust of the clinicians and nurses in pharmacy capabilities, as well as in information technology. For the transition to the new EHR, there was an intentional and substantial focus on pharmacy as well as clinician workflows. The physician informaticist ensured that order sets would reflect local physician workflows, and the pharmacy informaticist ensured that convenience order sets would facilitate pharmacy workflow including validation and clinical review of the medication orders. The pharmacy department allotted 20 h of training time for each pharmacist incorporated as part of their routine daily workflow.
