If good coordination is available, then patients’ quality and safety will be assured; also, patients will have a positive experience related to the nursing transition. Much dissipation occurs when miscommunication and discontinuity occur in nursing transition. This dissipation can be in the form of dangerous medical and nursing errors and unnecessary repeated referral. Every hospital should develop a clear referral guide, as well as compete to increase the quality, so it can be trusted by other health service providers to refer their patients there. If the hospital has had a good reputation through Lean implementation, then it possibly becomes a referral for other hospitals to send patients to. One of the Lean implementation purposes is simplifying the referral process in the outpatient unit. Lean steps which can be implemented involve: Learn to See, value flow mapping, eliminate, combine, reduce, simplify cycle, and plan, do, study, act cycle.
