ABSTRACT
Case history A 58-year-old man is admitted to his local hospital with a progressive history of shortness of breath. He has felt this way for some weeks, but over the last few days he has become increasingly breathless. He denies any chest pain or palpitations. He claims to have been fit and well up until the last few months with ‘just a touch of diabetes’ as his only problem.
