ABSTRACT

History A 48-year-old Caucasian male was referred to cardiology with a history of dyspnea and palpitations.Thallium stress testing showed no evidence of ischemic heart disease (IHD).Total cholesterol was found to be 8.4 mmol/l. He was referred to the Lipid Clinic where the abnormal lipid profile was confirmed.Total cholesterol was 7.9 mmol/l, triglyceride 2.3, calculated LDL-cholesterol 5.85 mmol/l and HDL-cholesterol 1.0; fasting glucose was 6.1 mmol/l. Glucose intolerance had been diagnosed 2 years previously with a 2-hour post glucose load value of 8.4 mmol/l. He had stopped smoking 15 years earlier and enjoyed a moderate alcohol intake of 21 units per week. An only child, his father developed IHD in his late sixties and at the age of 74 underwent coronary artery bypass grafting. His mother is alive and well in her mid seventies. A paternal aunt is known to have diabetes mellitus.
