ABSTRACT

Hepatojugular reflux (9) is a useful test in patients with right-sided heart failure, as hepatomegaly may occur in right-sided heart failure and venous congestion. The patient lies down in the same position as for JVP measurement, keeping the mouth open and breathing normally (to prevent the Valsalva manoeuvre, which may give a false-positive test). Moderate pressure is applied over the middle of the abdomen for 30-60 seconds. The test is positive if the JVP increases by at least 3 cm and is maintained during the period of compression.
