Nevertheless, there are certain points of controversy that should be clarified, starting with the simple fact of establishing more precise indications. In addition, a multitude of more specific technical details should be discussed, including how to perform adhesiolysis, how to manage the hernia sac, the postoperative seroma, the type and size of the mesh, and how to insert and secure the mesh. One of the most interesting points currently being debated is whether it is necessary to use sutures or tacks. We have developed a laparoscopic procedure without sutures for the repair of ventral hernias, a technique we have come to call the ‘double-crown’ technique.
