Management First the wound must be sealed either by surgical closure or temporarily by an occlusive dressing, so converting the open pneumothorax to a closed pneumothorax. A chest drain is then inserted. The danger in this is that, before the chest drain can be inserted, the open pneumothorax may be converted to a tension pneumothorax.
