Results Seven refinements of Millard’s rotation-advancement technique are presented, which he noted after criticizing his own postoperative results over an 8-year period. 1. The first is lengthening of the unilateral short columella with the C flap. He sug-

gests holding up the affected nostril arch to approximate the unaffected side in height. The resultant defect at the base of the cleft side columella indicates the amount of tissue deficiency that is needed. This defect may be filled with the C flap.
