Transfer of the tissue as a free flap offers additional advantages that may eventually make this technique the preferred method of transfer of the lower abdominal tissue for breast reconstruction.1

Research Question The authors studied the transverse rectus abdominis myocutaneous (TRAM) flap in the setting of immediate breast reconstruction, which was a novel idea at the time. After the first successful transfer of the TRAM as a free flap for immediate reconstruction,2 the authors proposed certain advantages for the microsurgical flap.3 This paper compares the conventional versus the free TRAM flap for the very first time in the literature.
