The Accident at DSM: Learning from a Major Accident in The Netherlands

This paper deals mainly with the accident at DSM (the Dutch Mining Company) on 7 November 1975 in the Western Mining District (of the province of Limburg in the south of The Netherlands), in which 14 employees lost their lives.
