What Is Requirements Risk Management? Poor requirements engineering practices have led to some spectacular failures. Bahill and Henderson (2005) analyzed several high-profile projects to determine if their failures were due to poor requirements development, requirements verification, or system validation. Their findings confirm that some of the most notorious system failures were due to poor requirements engineering. For example, the Titanic in 1912, the IBM PCjr in 1983, and the Mars Climate Orbiter in 1999 suffered from poor requirements design. The projects Apollo 13 in 1970, Space Shuttle Challenger in 1986, and Space Shuttle Columbia in 2002 all failed because of insufficient requirements verification (Table 5.1).
