ABSTRACT

Investigation is the process for obtaining facts and information for a better understanding of an event or to determine the cause of an event. Safety investigations involve fact ‘nding for the purpose of identifying safety breaches, loss, and safety concerns. Compliance is required in 29 CFR §1904.0, “Recording or Reporting a Work-Related Injury, Illness, or Fatality.” An investigation may expose activity violating Occupational Safety and Health Administration (OSHA) regulations, company policy, criminal laws, and administrative regulations. For example, an employee falsifying company records may be disciplined by the company and be subject to criminal penalties. The company may face a ‘ne, a lawsuit, or administrative sanctions for the activity.
