ABSTRACT

History A 56-year-old male with type 2 diabetes mellitus presented with a 2-3-week history of lethargy and ankle oedema with watery diarrhoea for the previous ten days. In addition, he reported that his blood glucose had been about 22 mmol/L and that this had been associated with thirst and urinary frequency. There was no history of shortness of breath or chest pain. Current drug therapy was furosemide, metformin, rosiglitazone, atorvastatin and dihydrocodeine. He took no alcohol and had nearly 90 pack-years of cigarette smoking until stopping about two years previously.
