ABSTRACT

History A 45-year-old Caucasian male has been referred to respiratory outpatients. He has a 3-month history of worsening shortness of breath with an associated non-productive cough, which is more severe at night. He feels generally lethargic and achy, and has also experienced sweats and shivering at night. His weight is steady. He has also noticed some painless swelling of his cheeks. He is married with two children and works as a solicitor. He does not smoke and drinks only socially. There is no other history of note.
