ABSTRACT

History A 62-year-old woman presents with a 4-week history of mild non-specific band-like epigastric pain radiating to the back. The pain seems to be worse at night with no relieving factors and is dull in nature, but constant. She also complains of weakness of her lower limbs over the last week prior to admission, which has been progressively worsening. The only other relevant history she gives is a bout of diarrhoea and vomiting 2 weeks ago, which settled without treatment. On direct questioning, she denies symptoms of bladder or bowel dysfunction, weight loss, loss of appetite, weakness, pins and needles or numbness of the upper limbs and denies other symptoms of note. She is a non-smoker with minimal alcohol intake and no past medical history.
