Examination He is found to have a pulse of 110 bpm regular, blood pressure (BP) 100/50 mmHg, jugular venous pressure (JVP) at 6 cm above the sternoclavicular joint, a strong apical thrill with a loud pan-systolic murmur throughout the precordium. The respiratory rate is 20 per minute and the chest is clear. The upper abdomen is tender, but there is no guarding and bowel sounds are normal. He is referred for a medical review after the blood results are obtained. The medical registrar has reviewed the history and elicited a history of some vague chest discomfort about a week prior to admission.
