ABSTRACT

In the absence of contradiction of the technique, I would select a spinal anaesthetic with a small-gauge pencil-point needle (ideally a 27-gauge Whitacre needle). I would use approximately 2.4-2.6mL of bupivacaine (heavy) along with 200g of morphine with a 1000-1500-mL crystalloid preload. I would follow this by testing the sensory block and managing haemodynamics as appropriate, with monitoring of respiratory function up to 24 hours postoperatively.
