ABSTRACT

A 36-year-old man presents to the surgical outpatient clinic with a 3-week history of soiled underwear. He is known to have Crohn’s disease. On examination, an opening is seen at the posterior margin of the anus which is discharging faeculent material. Subsequent imaging demonstrates an anal fistula that tracks through the puborectalis muscle.
