A number of theories have been proposed over the years to explain the very odd occurrence of fat within the nervous system. Ehni and Adson in 1945 suggested origin of intracranial lipomas might be due to:
Metaplasia of pial cells or undifferentiated cells surrounding blood vessels have also been proposed, particularly to explain lipomas occurring in locations that are not mid-line and those with perivascular components. Truwit and Barkovich presented an eloquently reasoned discussion of these possibilities, and argued that the origin of intracranial lipomas was from the abnormal persistence and maldifferentiation of the meninx primitiva during the development of the subarachnoid space (Truwit and Barkovich, 1990).
